Team observations (TO1 & TO2)

Trainees need feedback from a range of healthcare professionals and the TO1 form is to be used for this purpose. This multi-source feedback tool will form part of your assessment and will inform the ARCP/RITA.
The process involves:
- Trainees agree with the Educational Supervisor /College Tutor who the TO1 forms will be sent to.
- Trainee sends out the forms, advising that the forms be returned to Educational Supervisor/ College Tutor.
- The trainee does not collect them
- Educational Supervisor / College Tutor collates them to form the TO2 and discusses the feedback with the trainee. This will include the free text comments.
- All TO2s must be kept in the trainee’s portfolio and will inform the ARCP process
Trainees have the right to view TO1 forms
Your educational supervisor may also send out additional TO1 forms as and when required. TO1 forms should be completed twice per year, one of which should be organised to be ready for your educational supervisor meeting before your ARCP/RITA.
A minimum number of 10 TO1 forms should be collated (send >10 to ensure adequate response)
- Include current supervising consultant(s) – at least 3 consultants
- Senior members of nursing and midwifery in clinical areas – antenatal setting, labour ward, gynaecological wards and outpatient clinics, theatres – gynaecological and obstetric
- Trainees – maximum of 3 (at their level, above and below)
- Staff from other specialties outside of Obstetrics and Gynaecology – must be senior medical staff
If a trainee receives a poor TO2 score (which includes either “unsatisfactory”/ “improvement needed” or significant negative comments in the free text sections) the Educational Supervisor / College Tutor should discuss how the trainee can address the issues identified and additionally should alert the Training Programme Director.
TO2 forms should generally be taken together with other evidence in determining the outcome of ARCP, however a particularly poor TO2 could, by itself, suggest an ARCP outcome 3.
