Ambulatory gynaecology may be an unfamiliar term to many gynaecologists working outside the USA. However, the term is increasingly being adopted in the UK to describe a ‘see and treat’ management philosophy in outpatient clinics in combination with minimal access surgery in the day surgery unit. In recent years, the Royal College of Obstetricians and Gynaecologists in collaboration with the British Society for Gynaecological Endoscopy has run scientific meetings dedicated to ‘ambulatory gynaecology’. The adoption of the concept by international and national societies will help promote the management philosophy. This move is timely because a change from traditional care pathways to more cost-effective, patient-centred approaches to medical practice lies at the heart of modern health service management. In light of this we decided to produce a book that sets out how the concept of ambulatory gynaecology can be applied to the main areas of gynaecological practice.
It is hoped that after reading this book the reader will be stimulated to challenge the more traditional models of service delivery in gynaecological practice.

This Report, the sixth of the series of triennial reports, covers maternal deaths for the UK in the period 2000–02 and is the first to be produced under the auspices of CEMACH (Confidential Enquiry into Maternal and Child Health). The methodology used for its preparation has followed the same pattern as previous Reports.
The Enquiry Reports have played an important part in the reduction in deaths attributable to pregnancy since 1952. A pregnant woman in the UK today faces less than a 1 in 19,020 risk of dying from obstetric complications directly related to the pregnant state. This is a remarkable 12-fold improvement when compared with 50 years ago.

Over the last two decades the practice of medicine has changed beyond all recognition. A doctor is no longer in a position to dictate management to patients and it is essential that all procedures are explained to patients and informed consent is obtained. Clinical photography is subject to the same rules and regulations and it is no longer the prerogative of the clinician to display clinical photographs without a patient’s consent. Most readers will be familiar with the textbook edited by JB Lawson and DB Stewart entitled Obstetrics and Gynaecology in the Tropics and Developing Countries. Published in 1967, it met a great need and remained in print in the English Language Book Society paperback version until 1991. Before then, parts of it had become outdated, the Safe Motherhood Initiative had begun and there was reawakened inspiration and impetus for better maternal health, which continues worldwide. These developments called for a radical revision of Lawson and Stewart’s masterpiece: the result is this book.

The large number of professionals involved in producing these Reports are driven by a desire to improve care. Fifty years ago, when this project began as collaboration between the RCOG and the Ministry of Health (as it then was), doctors knew that some women were dying unnecessarily. Today, the authors of this Report can still feel anger at the way the system continues to fail some vulnerable young women.
As well as highlighting problems the Report also illuminates successes. Great improvements have taken place over the years in, for example, the safety of obstetric anaesthesia and the treatment of hypertensive disease. The present Report records a remarkably low death rate from obstetric haemorrhage and a dramatic fall in deaths from thromboembolism after caesarean section, following publication of RCOG guidelines. After a decade in which further reduction of maternal mortality seemed elusive, the latter breakthrough shows how improvements in practice can still be made and can still save lives.
