Question: In a 50 year old with stage 1C Granulosa cell tumour, ascitic fluid negative but spontaneous rupture. No residual disease. Adjuvant chemotherapy or no chemotherapy?

Answer: Three recent reviews report that there are no clear conclusions regarding the role of postoperative chemotherapy in stage I disease, although the use of adjuvant chemotherapy or radiotherapy has sometimes been associated with prolonged disease-free survival and possibly overall survival (Geetha, Koukourakis, Pectasides). No more recent studies were identified.

Tumour rupture is a risk factor for recurrence (Geetha).

A protocol for a Cochrane review of different treatment modalities for the management of adult onset granulosa cell tumours of the ovary has been published (Shanbhag).
