Trainees Returning to Work

 

  • Arrange Absence from Work meeting at earliest date with Educational Supervisor (ES)
  • Complete Absence from Work form* (Word document 9kb)
  • Forward completed form to Training Programme Director (TPD) and Deanery Specialty Lead
  • Arrange Workplace Risk Assessment if applicable

  • Arrange Return to Work meeting with ES 6-8 weeks before return date
  • Discuss and agree learning needs and period of supervision
  • Complete first part of Return to Work form* (Word document 22kb)
  • Arrange KIT days and other methods of ‘keeping up to date’
  • Arrange Workplace Risk Assessment if applicable

  • Review of satisfactory progress after period of supervision
  • Complete second part of Return to Work form* (Word document 22kb)
  • Forward completed form to TPD and Deanery Specialty Lead
