From January 2015, every maternity service the UK will be asked to complete a brief online data collection form for each eligible incident that occurs under their care. The form contains questions related to the adverse event and the results of local serious incident investigations and root-cause analyses.
According to the RCOG’s Clinical Governance Advice document Improving Patient Safety: Risk Management for Maternity and Gynaecology, all Each Baby Counts cases should already be subject to a thorough internal investigation. The success of the project depends on clinicians undertaking high-quality, honest reviews of practice and sharing the lessons learned for national analysis.
