At University Hospitals Coventry and Warwickshire (UHCW) NHS Trust, we have adopted a multi-professional approach to the reporting of Each Baby Counts cases. We felt it was important that the reporting role was shared among our team and we have therefore nominated three lead reporters (Maternity Risk Manager, Consultant Neonatologist and Maternity Risk Officer).
Since the project launch in January 2015, we have reported six cases via the online system.
To ensure we effectively identify and report all our cases, we have added the requirement to report Each Baby Counts cases to our Clinical Incident checklist and local monitoring system. We have so far divided the workload by case definition:
- Our maternity risk management team reports all poor outcomes at delivery and cases of early neonatal death (up to 48 hours)
- Our Neonatal Lead Reporter is responsible for reporting cases of early neonatal death (post 48 hours) and cases of severe brain injury diagnosed after transfer to the NICU
In the future we aim to arrange a bi-monthly meeting where we will meet as group to audit our compliance with the project’s requirements and reflect on our experiences using the reporting system.
