NHS Resolution early notification scheme

NHS Resolution (formerly NHSLA) requires trusts to report all maternity incidents that have led to severe brain injury. This page sets out the process and links to the notification form.

Download the form

If you have any problems downloading the form, please contact NHS Resolution at ENTeam@resolution.nhs.uk.

Process

There is some overlap between the information requested by NHS Resolution and the information already being submitted to the RCOG Each Baby Counts programme. We are working with NHS Resolution to streamline the process, but in the meantime have agreed the following:

  • Please submit data to the RCOG Each Baby Counts programme as usual.
  • Please notify your legal services department within 14 days that a notifiable severe brain injury incident under the early notification scheme has occurred, using the NHS Resolution form. Your legal services department will then report the incident to NHS Resolution within 30 days of the incident.
