Planning an absence from work
- Arrange Absence from Work meeting at earliest date with Educational Supervisor (ES)
- Complete Absence from Work form* (Word document 9kb)
- Forward completed form to Training Programme Director (TPD) and Deanery Specialty Lead
- Arrange Workplace Risk Assessment if applicable
Planning a return to work
- Arrange Return to Work meeting with ES 6-8 weeks before return date
- Discuss and agree learning needs and period of supervision
- Complete first part of Return to Work form* (Word document 22kb)
- Arrange KIT days and other methods of ‘keeping up to date’
- Arrange Workplace Risk Assessment if applicable
Returning to work and assessing readiness to practice
- Review of satisfactory progress after period of supervision
- Complete second part of Return to Work form* (Word document 22kb)
- Forward completed form to TPD and Deanery Specialty Lead
