Female Sterilisation (Consent Advice No. 3)

Please note: This document should be used as part of the process outlined on our main consent page. The form should be used to help record women’s understanding of the implications of the final agreed procedure.


This document is appropriate for a number of procedures and combinations. The consent form should be carefully edited under the heading ‘Name of proposed procedure or course of treatment’ to accurately describe the exact procedure to be performed. If consent is required for additional procedures, further procedure-specific consent should be obtained.

The aim is to ensure all women are given consistent and adequate information for consent. The consent form should be used together with dedicated patient information. After discharge, women should have clear direction for obtaining help if there are unforeseen problems.
